Enrollment Fees

Coverage begins once enrollment fees are paid in full.

• Single Member: $367

• Additional Adult Household Member: $345 per person

• Dependents (25 and under): $215 per person

Important Policies & Eligibility

• Valid only at Collegeville Dental Center

• Available to individuals 18 and older; dependents include spouses, domestic partners, and children through age 25

• Cannot be combined with dental insurance or other offers

• Discounts apply only when payment is made in full at the time of service

• Membership term is 12 months from enrollment date

• All payments are nonrefundable

Certain exclusions apply, including reduced discounts on veneers, Invisalign, and orthodontic treatment, as well as services requiring referral to a specialist. Treatment fees are guaranteed for three months from the date quoted.
